Before you download and install HFW-4.91.1-PS3UPDAT.PUP, ensure the following:

On the USB drive, create this exact path:
PS3\UPDATE\
Then place the HFW-4.91.1-PS3UPDAT.PUP file inside the UPDATE folder.
Important: Rename the file to exactly PS3UPDAT.PUP (all caps). The PS3 will ignore any other name.

Backup Your Data:
Go to Settings > System Settings > Backup Utility and back up to another USB drive. Installing HFW is safe, but a power outage during flashing could corrupt the system.
Disable System Updates:
Go to Settings > System Update > Automatic Update and turn it off. This prevents Sony from quietly downloading OFW 4.92.
